Event Description

A $157 million funding opportunity to build alignment of clinical and community services at the local level was announced by the Center for Medicare and Medicaid Innovation (CMMI) in January. The Accountable Health Communities (AHC) Model will support up to 44 communities to screen patients for health-related social needs and connect them to community services. Applications are due on March 31, 2016.

What does this opportunity mean for eligible organizations and how can communities leverage this grant to support population health?

Our Web Forum will consider these and other questions, drawing from national, multi-sector efforts underway in which healthcare and communities have linked clinical and community services to improve population health and enhance health equity.

Our panelists will discuss:

  • Promising approaches to screening for social needs;
  • Connecting patients to community services;
  • Managing a multi-sector coalition; and
  • Data collection and evaluation.


You’ll hear their lessons learned and have the opportunity to ask them your questions.
