A $157 million funding opportunity to build alignment of clinical and community services at the local level was announced by the Center for Medicare and Medicaid Innovation (CMMI) in January. The Accountable Health Communities (AHC) Model will support up to 44 communities to screen patients for health-related social needs and connect them to community services. Applications are due on March 31, 2016.
