As our nation continues its march towards the successful implementation of health care reform and payment reform, the basic infrastructure is being put into place, but many questions remain. Will the newly insured understand how to access and navigate the health care system? Will we be able to improve quality, control cost and drive utilization to these ends? Can we survive in a new era of accountability, with financial carrots and sticks driving population health, patient experience, patient safety, and transitions of care? How can we do all of this for an increasingly diverse, vulnerable patient population? Can we improve quality, achieve equity, eliminate disparities and deliver high-value, high-performance health care?

The Healthcare Quality and Equity Action Forum will provide the essential strategies to improve quality, achieve equity and deliver high-value healthcare to diverse populations. Our focus on access and accountability will allow us to tackle these issues head-on, sharing strategies for success. Leaders from multiple disciplines and health care organizations who work in the areas of quality and safety; disparities, diversity and equity; health policy; and health care design and delivery, as well as anyone interested in these fields, will find the Forum to be a unique and worthwhile experience.

Come hear from leaders on how to:

  • Secure leadership buy-in on equity in the C-Suite
  • Leverage big data to address disparities
  • Develop community-based approaches for addressing disparities
  • Prevent readmissions for diverse populations
  • Create health plan strategies to improve equity
  • Discuss the US Department of Health and Human Services and Centers for Medicare and Medicaid Office of Minority Health’s Health Equity Strategy
  • Understand America’s demographic transformation
  • Leverage disruptive and innovative strategies to achieve equity
  • Address disparities in oral health care
  • Deploy population health management approaches
  • Deliver high-quality care to dual-eligible beneficiaries

Network with colleagues and peers from hospitals, health plans, health centers, and other healthcare organizations that are integrating disparities solutions into quality improvement, and innovating at the front-line. 

The Forum promises to be a stimulating, action-oriented, and cutting-edge event where participants will leave better equipped to assure meaningful access and be prepared for accountability as we strive to deliver high-value health care to diverse populations.
