The San Francisco Department of Public Health (SFDPH) is accepting applications for one (1) full time position in class 2591 Health Program Coordinator II. This position will serve as a PRIME/QIP program coordinator to manage projects related to the Public Hospital Redesign and Incentive in Medi-Cal Program (PRIME) and Medicaid Managed Care Rule Quality Incentive Program (QIP) to make improvements in the delivery of health care across the San Francisco Health Network (SFHN). Working with the SFHN PRIME and QIP leadership teams, the program coordinator will focus on: 1. project coordination, 2. data analytics, and 3. quality improvement interventions.  The coordinator will manage projects to ensure that the project goals are met and completed according to schedule.

PRIME is the current Medicaid 1115 waiver’s 5-year, $160 million pay-for performance delivery system redesign program.  It consists of 10 distinct initiatives with 57 measures that cover 3 domains:  outpatient delivery system transformation and prevention, high-risk/high-cost populations, and resource utilization efficiency.  The Medicaid Managed Care Rule QIP is a new value-based payment program with 26 measures across 4 domains: primary care, specialty care, inpatient care, and resource utilization.  Together, these programs aim to improve the quality and value of care provided across California's safety net hospital systems.

SFHN is the vertically integrated, public safety net delivery system for the City and County of SF. SFHN clinical services include Zuckerberg San Francisco General Hospital (ZSFG), an acute care hospital providing emergency, inpatient, and trauma services as well as comprehensive specialty and diagnostic services; Laguna Honda Hospital (LHH), a long term care facility; and an Ambulatory Network that consists of 14 hospital-based and community-based primary care clinics, an extensive network of behavioral health services, maternal child health services, and jail health services.  

LOCATION: 30 Van Ness Avenue, San Francisco, CA 94102.

WORKING HOURS: Full-Time, Monday to Friday, business core hours 8:00 a.m. to 5:00 p.m. Persons employed in this class will work weekdays. Shift assignments are subject to changes depending on the operational needs of the division. This position may require occasional work during the evenings, nights, weekends, and/or holiday shifts.


Under general direction, the 2591 Health Program Coordinator II will be responsible for a wide range of administrative duties that support the Office of Contracts Management and Compliance. The 2591 Health Program Coordinator II will perform the following essential duties:

  • Project manages assigned initiatives and programs, developing project plans to monitor progress.
  •  Ensures that projects are delivered on time and within budget.
  • Identifies tasks and sequencing, develops project plans, and estimates schedules for deliverables against program needs. 
  •  Manages relationships between multiple stakeholders, across departments and external vendors. 
  • Attends, coordinates and facilitates project team meetings, including taking meeting minutes, creating agendas, and following up on action items and assignments.
  • Prepares and tracks performance measures (e.g. financial/clinical/operational) including data metric review, validation, and visualization of data metrics in dashboards to manage ongoing program performance. 
  •  Prepares reports to support assigned programs.
  • Provides guidance for assigned clinics/programs on the development, testing, and implementation of standard work.
  • Assists in creating training materials to facilitate new workflows and improvements and trains staff on implementation.
  • Develops performance improvement plans using Lean or other performance improvement methodology.
  • Supervises and oversees professionals and paraprofessionals who are engaged in the assigned projects, including reviewing assignments and plans with staff and providing instructions.

The 2591 Health Program Coordinator II also performs other related duties as assigned.
