Background

The goal of the Alameda County Health Coach Program is to improve the health of community members by pairing them with culturally sensitive health coaches trained in chronic disease management. Health coaching is an innovative method to improve health outcomes by motivating patients to manage their health. Health coaches assist patients in understanding their health plan, making medical appointments, and advocating for themselves. Mentoring in Medicine’s Community Health Fellows Program provides youth with leadership development, tiered mentorship, and health education that allows them to work in their communities raising awareness about health inequities that disproportionately affect disadvantaged communities.

Position Overview

The Health Education Trainer (HET), under the direction of the Health Coach Program Director, is responsible for the overall health communication activities and development of health coach training curriculum. This curriculum will be taught by the HET to train Alameda County health coaches and Mentoring in Medicine & Sciences program youth on various health education topics and behaviors that promote health and wellness. The HET is also responsible for developing workshop curriculums that address gaps in nutrition, disease education & wellness resources in Oakland and surrounding communities. This is a wonderful opportunity for a person interested in supporting efforts aimed at improving the health outcomes of their community. The successful candidate will have an interest in and enthusiasm for providing health education and addressing the health disparities of underserved communities.

Key Areas of Responsibilities

  1. Primary duties include tailoring appropriate health education trainings and facilitating individualized behavior change interventions to meet patient needs.
  2. Provides Health Coaching training to addresses healthy behavior change, including stress reduction, smoking cessation, food and exercise, and other wellness behaviors.
  3. Planning, implementing, and evaluating health education programs to prevent illness and promote the health of the community.
  4. Develops and deliver workshops to promote general wellness and stress management for students.
  5. Designs and delivers outreach projects on other topics, which may include violence prevention, alcohol prevention, and other relevant college health topics.
  6. Identify new and existing resources and organizations that serve student / patient health needs, develop and maintain community partnerships, and organize and assist with educational activities focused on health and clinic promotion.
  7. Participate in events/trainings/presentations with community partners and schools.
  8. Provides assistance with team projects and/or activities and additional administrative support.
  9. Other duties include case management, managing data collection, data entry, and contributing to program evaluation.

Minimum Qualifications

  • Two years or more experience in health education or Bachelor’s Degree in public health, human services, health sciences, psychology, sociology or related field
  • Knowledge of behavior change strategies; training in health coaching; experience with providing health coaching preferred.
  • Skills associated with the development and delivery of health education programs and services.
  • Working knowledge of the health promotion and public health field.
  • Experience with, or ability to develop, outreach work in alcohol prevention, violence prevention, and other relevant college health topics.
  • Experience with working with underrepresented/underserved communities; experience working with college-age populations preferred.
  • Effective interpersonal skills needed to work with diverse populations.
  • Ability to communicate effectively both verbally and in writing.
  • Must be 18 years-old or older
