Grant funded full-time position for 10 months with possibility of continuation
The program associate will work on a New York City Department of Health funded initiative to expand opioid overdose prevention education at Montefiore Medical Center. The program associate will be an integral member of the Naloxone Consult Team at Montefiore Moses Hospital and will lead overdose prevention and naloxone trainings for patients and providers. The program associate will utilize screening tools to identify at-risk patients, distribute naloxone kits to at-risk patients prior to hospital discharge, follow protocols for care coordination and documentation, and collect and manage data for program monitoring and quality improvement. This position will be supervised by the program director of the Montefiore Primary Care Opioid Overdose Prevention Program, and is expected to assist with other opioid overdose prevention related projects and collaborate with various clinical and administrative staff involved with the program. The program associate will also need to travel locally in New York City to attend trainings and meetings required by the Department of Health.
