OverviewThe Project Specialist will primarily support the Center for Population Health strategy for Partners primary care practices to achieve and sustain primary care transformation in the Patient- Centered Medical Home (PCMH) model. This person will educate physicians, administrators, and staff on the benefits and challenges of PCMH and support the implementation of this model.

Principal Duties and Responsibilities
• Educate physicians, administrators, staff and other key stakeholders about the components of the PCMH model and the benefits and challenges of transitioning to that model of care.
• Consult with practices about their needs related to PCMH and support the initial National Committee for Quality Assurance (NCQA) PCMH recognition/application process.
• Support practices with NCQA PCMH annual reporting needs.
• Develop and maintain relationship with NCQA to support PCMH team needs.
• Develop and maintain strong working knowledge of Epic functionality, specifically related to workflow development and reporting to support primary care practice needs.
• Manage internal team strategy for sharing best practices to support the adoption of PCMH model. This will include monitoring approved documentation, best practice examples, tip sheets and other relevant information and communicating this to PCMH team and RSO (Regional Service Organization) leadership, as appropriate.
• Support projects for other evolving strategic imperatives at the Center for Population Health’s including the Medicaid ACO (Accountable Care Organization) and patient experience of care.
Qualifications
• BS/BA Required.
• 3 years of relevant work experience.
• Experience with MS Office applications, including Word, Excel, PowerPoint, and Access.
• Experience with Epic and other electronic medical records is a plus

• BS/BA Required
• At least 3 years of experience working with physicians and primary care practices in a business-related capacity.
• Excellent communication skills
• Excellent ability to manage multiple projects
• Ability to foster and maintain relationships with external stakeholders
• Solid understanding of the health care environment
• Strong project management skills
• Ability to work independently
• Position requires frequent local travel to physician practices


Professional office setting located in Assembly Row, Somerville, MA. Requires travel locally to other Partners’ sites. Reliable access to transportation to travel between sites for business purposes is required.
